RSA sign and decrypt

I have a question regarding RSA sign and decrypt operations.

Is the output of RSA_SIGN(X,K) equal to the value or RSA_DECRYPT(X,K)?
K is the key and X is random number.

does the sign operation include hashing X and then sign it?
