It works very easily. One simply manufactures an obscenely large batch
of chips containing one each of the number range ... no duplicates.

The numbers are not assigned by a central control other than at the
point of order (someone has to pay for all this stuff whichever method
is used) when the instruction is 'make a set of chips carrying numbers
from 1 to n-mega'. They are then distributed to hospitals for
implantation into the victims, I mean 'population'.

These are implanted (as is done with other animals) at birth by the
hospital / midwife etc.

Every person will thus have an ID which is unique. Any person found not
to have a chip implanted will be arrested and taken to the nearest
hospital where a chip will be implanted.

Anyone having their chip removed will be shot.

When a person dies, their chip is removed and vapourised ... there would
be quite a market for used chips I would think.
