PIN block composition

I have few question not directly connected to cryptography, but I
believe some cryptographers will know.
Can anyone tell me how is a PIN block of a credit card composed?
How is it compesed when the PIN is 4 or 12 digits?
Can the CVV be a part of the PIN block?
