I've run into the exact same thing on two of my XP installs. Interestingly
enough, the problem hasn't appeared for me on new installs, but starts
showing up after the system has been in operation for some time (weeks). So
far rebooting seems to be the only 100% solution, although logging off and
back on *sometimes* works. For unknown reasons, file status isn't being
properly/completely refreshed... AutoCAD files have been the worst culprit
for me.
