It is probably disappearing because another account has
been defined.
At a login prompt, with no users switched out, try pressing
alt-cntrl-del twice and then manually logging in with the
account info typed in.
If this works, it is then just a matter of making sure the
account will remain displayed. This can be done with
TweakUI or by editing the registry.
