Could it be that another user has fiddled with the settings to try to permit
access?
I do not think Content Advisor is a very reliable or satisfactory control
method, since it relies on each web page author to voluntarily add a code to
the web page that accurately describes the content on the page. [You could
block pages that have no rating, but that is probably unsatisfactory as
well.] I would disable it and consider a third party program such as:
