You are correct about KB936181 Mike, except the issue is not limited to Vista : Security Update for Microsoft XML Core Services 4.0 Service Pack 2 (KB936181) may repeatedly appear in the update list on Microsoft Update or on Windows Update

Applies to Automatic Updates as well even if the KB doesn't mention it.

Uninstall the update. Then find the update on MS's web-site and download the manual installation package and install it from there. I have a couple of installation packages that I had to do this for. It appears the Vista updates are not as rock solid as the XP updates.
