case study for s/c

Hello,
have somebody a usefull link to a case study or someone's practical
experience with deploying smart cards in an organization about 500 to 1000
employees wide?
I am especially interested in procedures and experiences such as:
- distribution of cards to new employees (who and what kind and number of
people creates the cards)
- policies to prevent card loss and real world estimate of its rate
- real world experience with physical card's wearing and some estimates
about how often a card have to be replaced due to normal abration or so
- the number of spare cards one have to have for a specific amount of people
- procedures for provision of temporary cards for temporary workers, for
those who left their card at home etc.
- how to allow access to a remote worker who lost/destroyed his card
- what other daily tasks will have administrative personel to do to maintain
user access
