Is this as seen in NTFS permissions dialog, in the IIS security
dialog, or in the Sharepoint/FPSE interface ?

Sharepoint health check and repair is in my opinion
terminally broken, and always has been since the day
it shipped. I once did a test server, scrambled the NTFS
permissions, did a full FPSE check and allowed it to do
all that it wanted to repair. I then immediately did another
check and it still said it needed to repair. It never did
converge, and it did plug in the Network and Interactive
all over the place (as usual).
I suspect this is your culprit.
Try not allowing it to run, resetting your grants, and
see if they stick.

Using a Domain Admin is not your solution.
Rather, make sure that the NTFS permissions on the underlying content
and the FPSE internal grants both allow the account used.
