Creating a registration authority

I would like to create an RA in my DMZ to send requests to an issuing CA
internally. How do I do this on a non-domain computer?
Also, is it possible once a user enrolls with a CA via autoenrollment to
deny another request from the same user. So, one cert per user until an
admin intervenes manually.
