Are you using the web interface? If so, (if I remember correctly):
1) Go to the web interface for certificate services
(http://localhost/certsrv/)
2) Select 'Request a certificate'
3) Select 'Advanced certificate request'
4) Select 'Create and submit a request to this CA'
5) Fill in all the details, and set 'Type of Certificate Needed:' to 'Code
signing certificate'
Then carry on as usual.
