As Jeff indicated, ISA Server is more than a stateful firewall. A few of
its features are web proxying, intrusion detection, client access based
in Windows group membership (i.e., authenticated access), forward and
reverse caching, server chaining, application level filtering, dynamic
and static packet filters, publishing, multiple ISAs in arrays, H.323
gateway, etc. There are also vendors that offer add-ons for traffic
virus inspection, anti-spam, IM monitoring and what not:

So, doing NAT and restricting ports is the least you can do with ISA ;)
