Following a posting to the NTBugTraq, and recently confimed by MS-
There is an issue with update.exe that does exactly what you are describing
if the user account running the patch doesn't have debug permissions. Added
that permission via group policy fixes the problem. (MS is aware and has
fixed the bug in newer versions)
