Sounds like you only need one field right - which can contain both the question and the answer.
I'm guessing you won't mind if the field to be held in plain text. Not that it really matters, since you can just encrypt the data before writing it into AD. I assume you'll write some kind of front-end interface to it for your Helpdesk to access the field right - like a webpage??

I'd suggest you look at the existing AD schema and pick a field that you don't currently use. AD is full of such fields for eg .. an EmployeeID field which is not visible in any MMC Snap-In. You just need to find one that accepts the characters you need and has the size to manage your requirements (in terms of max characters that will fit in the field). If you have Exchange 200x you'll have about 12 or 13 custom attributes fields which are free for you to use however you want.

I wish to add a security phrase and response to the schema. But before I add
my own schema keys I was wondering if there is already a schema key there for
a security phrase. And just to be clear I want something along the lines of

Whats your favorite color or Whats your security word and then a field where
the security word is held in Active directory for each individual user.
