The most likely cause for this is clients not meeting the pre-requisites
for this patch:

 Microsoft Data Access Components (all versions)
Prerequisites

This security update requires that you have any one of the following MDAC
versions installed:

MDAC 2.5 Service Pack 2
MDAC 2.5 Service Pack 3
MDAC 2.6 Service Pack 2
MDAC 2.7
MDAC 2.7 Service Pack 1
MDAC 2.7 Service Pack 1 Refresh
MDAC 2.8

The recommendation is to install MDAC 2.8 and then MS04-003 so that you
have the latest version of MDAC. If this is not feasible, you can check
your current version of MDAC using the following article and at least
update that version to a supported service pack.
