Certificate Services Online Request form

I am having problems with the online certificate request feature of
the Certificate Services. I'm trying to request a certificate through
the advanced request option. When I get to the form, a message
appears stating that an ActiveX control is being downloaded. The
problem is the download never seems to finish. The CSP drop down list
says "Loading..." and the hash algorithm drop down list is blank. I
had to reinstall my Certificate Authority due to an unexpected error
and I'm wondering if I could have missed something in the
configuration (although setup is pretty straight forward) or if it's a
setting in my browser. Does anyone have any ideas?
