The db_ddladmin, db_datareader and db_datawriter roles should be sufficient.

I have several SQL Server 2005 databases on my server.
What SQL Server 2005 security role has the least priviledges to complete the
following capabilities?
Able to create tables, triggers, create primary and foreign key
relationship, insert, update, and delete data in tables?
