Howto log which applications that is used

I have a database that is accessed through an Application Role with a password only know by my application.

Since the user 'sa' also can access the database I would like to now when this is done.
Reason: If I can see which user (and when) that is accessing the database without using the Application Role I can shout down my application.
This will prevent the customer to add users to the database with read permission since the application will "stop working"

Any ideas?
