    I have yet to trace such email to "someone I know". The best I can
    determine is that the sender is a "friend of a friend".

    To apply your example to a common situation:

    Your friend Bill forwards a humorous email to six of his friends, including
    you; he use the common "Cc:" email list, so now your email address is on
    his friend's computer in the message store of the email client. Bill's
    friend is a touch careless, and got his computer infected with a mass
    mailer worm. This worm constructs an email with it payload, and pulls your
    email address from the message store to use as the sender. Bill isn't
    infected, his friend is. But the poor recipient's email provider only knows
    the putative "sender" email address, yours, and sends you the Delivery
    Failure Notice.
