With anti-virus app disabled and all IE windows closed, Uninstall it via
Add/Remove Programs (will be listed under 'Windows Update', not 'Internet
Explorer'*) and Reboot.

Download MS04-040 again, if necessary, via Windows Update. Prior to
installing, disable anti-virus app and close all application windows.
