Looks like a application is causing a problem. Check the path
C:\WINDOWS\addgh32.exe
and see what that file "addgh32.exe". I think it's some kind of worm or
trojan file. Go to Task Manager and check for the processes, is addgh32.exe
running there? If yes, than try to kill that process and then see what
happens. If you think that it is a non important file, than try deleting it
and see if that solves the problem. Next time when you get a box saying
"send the report", than send the report and see if a windows pops up giving
you any advise on what might be the problem.

Hope this helps. Make sure you backup important data before deleting
anything in Windows folder on your system.
