This is not entirely true in all cases.
E.g. updates for Internet Explorer are usually cumulative and
supersedes previously released updates.
Also some of the latest updates supersedes some previous updates,
e.g. MS04-011/KB835732 supersedes MS04-007/KB828028, see
http://www.microsoft.com/technet/security/bulletin/ms04-011.mspx
for a table listing what other updates it replaces.
Also MS04-012/KB828741 is an update that replaces another updates,
as the name indicates:
Cumulative Update for Microsoft RPC/DCOM (828741)
http://www.microsoft.com/technet/security/bulletin/ms04-012.mspx
It replaces e.g. MS03-039/KB824146 (and MS03-039 replaced MS03-026).
