SpamPal (free). Also has plug-ins (all free):
Bayesian (for statistical weighting of words rather than stupid word
lists).
HTML-Modify (eliminates nasties from HTML-formatted e-mails).
RegEx (regular expressions).
Quarantine (should you want to keep text-only versions of spam-marked
e-mails, like to report them).
URLbody (detect spam based on URL in body to spammer's web site; they
eventually have to get you to their site despite all the obfuscation and
word hiding).
I can't tell you an exact percentage of spam eradication. After
enabling my ISP's spam blocking, using SpamPal and plug-ins, and a dozen
rules, I get about 2 to 5 that leak past per week. I don't bother
counting how many got killed (but used to get 30 to 60 per day before).
