Happens at my place all the time. 3 reasons for this... 1- user is logged
in to multiple machines, password is changed and at least one of those
machines is still logged in with the old password. 2 - user has a service
running on some machine with their ID instead of a system ID and it has an
older password or 3 - user is logged in to a terminal session with an old
password or didn't end it properly.
