Accounts getting locked out after 1 invalid logon attempt

As stated in the title, some of my active directory user accounts are
getting locked out if the user makes one invalid attempt. My Domain security
policy is as follows:

Account Lockout Time = 60 mins
Account Lockout Threshold = 5 invalid attempts
Account Lockout Counter Reset = 15 mins

Any idea why this would be happening?
