what David said, plus:
In Control Panel | Network make certain Windows Family Logon (AKA Windows
Logon in Win95) is selected, not Client for Microsoft Networks. If CMN is
selected even a blank password will still cause the dialog to be raised as
system startup.
If you've just installed/changed a LAN adapter the network provider gets
"automagically" switched to CMN from Windows Logon.
