Is privacy set to medium also?
If it is, my only suggestion would be to play with
your settings to see if you can get the update (and to
only do this at the Microsoft site, and restore the
settings when you are finished there).
While you are trying to fix this (and to eliminate
one possible cause of the problem), perhaps you might be
able to run scans from one of the free online vendors,
for instance Trend's Housecall:
