    This sounds like you've got a corrupted TaskManager. Whether this is
    accidental or the result of malware, I couldn't say, but this is the sort of
    action you might expect from a program closing unexpectedly - they tend to
    leave their system tray icons around, and the system doesn't delete them
    until it has a chance to verify that the icon is detached from a valid
    process - i.e. when you mouse over to the icon.
