GINA not called in Terminal Server connection...

I'm having an issue with my GINA. It appears to work flawless on the
console on both Windows 2000 and XP, however none of my GINA functions
appear to be called when I access the machine from a remote machine
(not on console).

The log file states normal startup messages consistent with the
console but end at the load of the 1400 dialog. (It is the press
Ctrl-Alt-Del dialog.) Log on and off the machine through terminal
services does not write anything else to the file. However if I walk
over to the console, my own GINA dialog appears and the log file
reflects changes.

The machine is Windows 2000 and manifested the problem with SP3 and
SP4. The machine has the OS on it only. No third party software at
all (except my GINA).

In regedit the key's value is
HKEY_LOCAL_MACHINE\SYSTEM\CurrentControlSet\Control\Terminal
Server\WinStations\RDP-Tcp

fUseDefaultGina = 0

Any ides what is wrong?
