After you save the urlscan.ini, do you restart
IIS services ?

If yes, and you didn't see anythign in urlscanxxxxxx.log
then this is not cause by urlscan.

Next, what's the error sub code for the 401 request ?
check IIS log file too. 401 related to login and permission
issue, what authentication being used ? do the user has
read access for the .exe file ?
