One possibly silly question is when the numbers are failing, have you tried
hitting each shift key a couple of times in turn, and making sure that the
"Num Lock" light is on?

Some keyboards (especially through KVMs) have sticky shift keys, and others
aren't very good about keeping their NumLock mode correct.
