Dave,
A couple of things. First, check out your logs and determine what file is
being referred to here. Is it something legitimate that perhaps needs a
different ACL or is it some attack type scenario. Remember, the logs are
UTC so won't match the time shown in the Event Log without the proper
offset.
Second, go to either the Server properties or Web Site properties of this
machine's Internet Services Manager MMC snap-in and navigate to the Custom
Errors tab. Select the 401-1 item entry and edit it. Change the type from
File to Default. This will spit out a 401-1 response hard coded into IIS
rather than look for the file from IISHELP.
Third, you could copy IISHELP from some other machine but that isn't really
necessary. I run all my machines without these helper files.
Last, you could write your own custom response file and place it in the
401-1 entry in the Custom Errors tab.
