Recommended smartcards and readers

What do SmartCard users recommend for readers and cards? (mainly
interested in cards for crypto authentication and porting credentials
such as S-MIME and code-signing certificates).

What are your good/bad/ugly experiences with this type of technology,
particularly on Win2000+.
