storing documents external or in the database

Hello
if i look at the current DMS applications (imanage, groupwise, etc.) they
all store the metadata in a database and the document itself is stored as a
normal file (encrypted and compacted).
How can i make external files available on an intranet/intranet without the
security problem that everyone can load the file if you know the name and
location (http:\\localhost\dms\document.pdf for example).
I like to store a reference to the document in the database, but that
reference should not be available without some kind of security protection.
Any idea's???
