Question regarding keys and SSH

Key management - What it would take to setup key managment and who would
maintain the keys? What both free and Vendor solutions (SSH LTD and
F-Secure)exist for Key management? If we don't implement with key
management, how can we still be as
secure? In other words for user/server authentication are there
other models that work as well? <= I have related back that servers have
two ways to authenticate (certificate and keys) and users have @7
ways (passwords, certificate, keys, PAM, etc) but I was going to dig
up more on that as well.
Any ideas and help appreciated
