However if you are running a mailserver at your frontdoor then the ip address
of the system connecting to your server in order to pass on the mail message
will be known irrespective of whether any of the headers have been forged.
Many spammers will send directly or via open-relays hence these lists are
useful and do in fact block tons of spam. Content scanners (such as the free
SpamAssassin) are a relatively recent development whereas these lists have
been successfully used for quite some time.
Many ISPs also publish the ip addresses assigned to their dialup connections
on lists such as MAPS DUL. This allows you to block direct connections from
such systems only accepting mail from the ISP's mailservers which will
hopefully have been virus scanned. (Although it is possible for an ISP to scan
outgoing mailthrough their mailserver for SPAM what they should do with such
mail is much more problematic than with viruses since unfortunately one
person's SPAM is another person's important mail message.)
Ideally you want to use both lists and content scanning.
