]How does the recipient know where in the one-time-pad to start?

You start the message with "I am using page 47".

Then you destroy page 47
The british printed them on silk and the agent cut away the line used
and burned it. (I am using silk 5 line 43)

]Another question.

]If more that two people are use he same pads, how does the sender know

oThey do not. A one time pad is used only once and by only one person.
Two people never use the same pad. Each agent is given his/her own .
The out and in pads are kept separate. (eg top half is out, bottom half
in).
