Yes, Sygate Traffic Logs contain all of those items.

It has time instead of time stamp. The log is for my use--not for the
use of others.

don't try to be an *** but is all this info easy to copy eg
can you just copy-paste instead of having to deal with sygate's own
proprietary ... log output format and *** like that ?
