Locked Accounts

I am having a problem once in awhile all my Accounts have been locked out. I
have it set in the policy to keep accounts lock till the administrator
unlocks. Why is it all the accounts are being locked instead just the
offending user, or is there another reason all the accounts have been
locked? Primary and Backup domain controllers are NT 4.0 with SP 6
