Two main protocols being used for this today.

SIP (Session Initiated Protocol) newer
        Kphone (http://www.wirlab.net/kphone/)

H.323 http://www.gnomemeeting.org/

Both work, just make sure ports are opened up on the firewalls and NAT
port forwarding is working if you're behind NAT.
