The DNAT HOWTO is your fwend(TM). See
<http://www.netfilter.org/documentation/HOWTO//NAT-HOWTO.html> esp section
6.2 for more. Make sure the packets are accepted on the INPUT or FORWARDING
chains as well, btw ;)
