Identifying Intrusions?

I was curious about tracking intrusions and distinguishing the difference between a legitimate user and a possible attacker on a LAN or WAN? I am curious how you would still be able to tell if an account has been compromised if the IP is spoofed and a MAC is spoofed both on a lan and a wan?
