Fortunately this kind of censorship is almost impossible. Your only bet
would be to completely disallow binary data through the server (there is
software out there that can do that) since pictures of explicit content
are - as of right now - indistinguishable from any other picture to
scanners. There have been a few attempts to generate and distribute md5
checksums of 'objectionable' pictures for filters, but a simple resize
or modification will render this approach useless.
As for explicit words - you'll have to come up with a list of these
words and feed them to http://www.antivirus.com/ (TrendMicro's) "Content
Filtering" engine.
Before you do this, you might want to have a look at
http://www.peacefire.org and read up on the efficiency and fals
positives associated with email and content filtering in general.
