These URLs should help. They got me up and running with my own CA server
for use with my OWA web server.

Subject: Problems with IIS and Certification Services
Hello. I have an IIS/5.0 on a lan and i have installed the Certification
Services on the same machine as standalone root CA. Now i can manage
certificates for browsers and mail programs... but i need a certificate
for my IIS/5.0 server. I run the wizard and it gives me just one
option, to send the request later. There is the option to send the
request to an online CA but it's disabled :(
Microsoft documentation says that i must use the wizard... but something
doesn't work.
Someone can give me some opinion and solution?
thannks :)
